P21 is a synthetic peptide analog derived from CNTF (ciliary neurotrophic factor), specifically designed to cross the blood-brain barrier and mimic the neurotrophic effects of CNTF without its antigenic properties. P21 contains four ciliary neurotrophic factor active region residues attached to a protein transduction domain (adamantane-based). It was developed to address the translational limitations of full-length CNTF, which cannot cross the BBB and elicits neutralizing antibody responses on repeated administration.

Mechanism of Action
P21 enhances neurogenesis in the hippocampal dentate gyrus by increasing brain-derived neurotrophic factor (BDNF) expression and promoting dendritic complexity of newborn neurons. It partially inhibits leukemia inhibitory factor (LIF) signaling, reducing the gliogenic fate of neural progenitor cells while favoring neuronal differentiation. In Alzheimer's disease models, P21 reduced tau hyperphosphorylation by modulating GSK-3β activity and decreased amyloid plaque-associated neuroinflammation.
